Standardized Patient Survey Templates

Purpose

PCSIG has developed a standardized patient survey template to support consistent data collection across organizations conducting patient-centric sampling initiatives.

Use of a common question set enables:

  • Comparable data across studies and sponsors
  • Benchmarking of patient experience
  • Aggregated analyses across broader populations
  • Stronger evidence to inform best practice

Organizations using the PCSIG survey contribute to a shared dataset that may be analyzed and published in aggregated, de-identified form.

Preferred Use: Online via Qualtrics

We strongly recommend using the official survey hosted on the Qualtrics platform. The online version ensures:

  • Standardized wording and structure
  • Version control
  • Consistent data capture
  • Ability to contribute to pooled analyses

Instructions for Researchers

Before using the PCSIG Patient Acceptability Survey, researchers must review the full Instructions for Researchers document. The guidance outlines:

  • The purpose and intended use of the survey
  • Which modules are mandatory, optional, or may be adapted
  • What wording and response scales must remain unchanged
  • How to tailor demographic categories while maintaining compatibility
  • Adverse event reporting requirements, where applicable
  • Reimbursement considerations and handling of personally identifiable information
  • How to share de-identified results with PCSIG to enable pooled analysis

Adherence to these instructions is essential to preserve standardization and ensure data can contribute to cross-organization analyses.

Downloadable Version

A downloadable template is available if required. However, modifications to wording, structure, or response scales may limit comparability and inclusion in pooled analyses. For this reason, use of the Qualtrics version is preferred.
